Any Landscapers or Gardeners out There?

I have a problem with my lawn. I live on a wooded lot and have a 15 year old zoysia lawn. I have a lot of what I would describe as pot holes in my lawn. I think these are from the trees decaying below that got cut down 15 years ago to create the lawn.

How do you fill those in? They are mostly 2-4 inches deep and in most cases a foot in diameter. But I have a couple that are 8-12 inches deep and a yard in diameter. If you tried to jog across my yard right now you would definitely hurt an ankle or a knee.

Is there a certain mix of dirt, fertilizer, sand? Help!
